CRITERIA


The Innovative Pharmacy Practice Award is designated to recognize outstanding patient care through innovations in pharmacy practice. One program is selected by the Healthcare Reform Committee (HRC) that best demonstrates the objective of the award, such as: demonstration of pharmacist or pharmacy technician role in the Triple Aim of improving healthcare quality, care/service, and cost through a model of care that focuses primarily on pharmacists and pharmacy technicians in the provision of patient care. Projects can be in a health-system, inpatient, outpatient, or ambulatory care pharmacy service area. Projects will be judged on demonstrating excellence in: improvements in quality, improvements in care/service, improvements in cost, or demonstrated role of pharmacist or pharmacy technicians in healthcare teams. Data collection period of three months or longer is strongly desired. The award is open to all pharmacists or pharmacy technicians, but at least one of the applicants from the team must be a CSHP member. The winner will be required to present a poster of their project at the CSHP Seminar.

June 1 st is the deadline for submissions.
